Do You Want A Bridal Makeup Trial Included In Your Contract?
Have you taken a look at my service and travel rates? (Service & Travel rates are located on the "Rates & Services" page of the website. Please take a look at it before submitting this inquiry, and if you have bridal party members that are paying for themselves please confirm with them that the rates are ok with them before putting them down. Rates will not be sent to you upon request since it is easily accessible online. Required
What is your preferred payment method? Required
By selecting "Matte," I confirm that I am ready to book my wedding date or trial and consent to receiving a contract via the email address "zaymodestybridal@gmail.com" with payment instructions for the retainer or balance for myself, my wedding party, or both. I understand that my date will not be secured, and my makeup services will not be booked, without a signed contract and paid retainer. If my wedding date is available, I will receive a contract via email within 24 hours of submitting this form. I will have 48 hours to complete and return the contract along with the retainer. After this 48-hour period, the contract will expire, and I must resubmit a request via this site, with no guarantee that my date will still be available, as priority may be given to subsequent requests. I confirm that I have read and understood the minimum booking requirements and agree to a $400 charge on my contract if this minimum is not met. By selecting "Glossy," I acknowledge that I am not ready to book within the next 48 hours and understand that I will not receive a response, as this form is only for brides who are prepared to book their trial or wedding date upon submitting it.
